If the green "Submit" button on the form does not work, configure Adobe Reader as described below. This will allow you to submit your form data to the saved forms list.
In the yellow information bar at the top of the form, click the "Options" button.
Then, click "Add host to Privileged Locations."
Note: To the left of the Submit or Help button, you may need to click the Red X or the Yellow Triangle to make the yellow information bar appear.
